Course Authorization

The first step to getting administrative approval is to fill out and submit the Course Authorization form. After receiving approval confirmation, you will be ready to develop your new Distance Education course.

Adobe Reader is required.

Using this form

• This file contains editable fields, allowing you to type in your responses directly on your screen.
• Fill out the form completely by placing your cursor at the beginning of the first line and typing your response. Use the tab key to move from field to field.
• To save your changes, choose Save As from the File menu in either Adobe Acrobat or Adobe Reader. Rename the file and save it to a convenient place on your computer.
