Distance Education Funds Request

The first step to getting administrative approval is to fill out and submit the Request for Distance Education Program Funds form.

The first page of the form contains instructions on how to complete the form and information on supporting documentation needs, form routing, decision notification, and timing considerations.

Note that once the form is completed you may save your changes by choosing Save As from the File menu in either Adobe Acrobat or Adobe Reader.

Adobe Reader is required.
